We are actively seeking Part-Time Community Integration Counselors in Williamsport, PA.
This role will be responsible for assisting individuals with learning physical, intellectual, emotional, and social life skills.
The Community Integration Counselor provides individualized services designed to assist participants in managing and overcoming the difficulties confronted when living in the community.
The pay-rate is $16.
75 an hour.
This position is Monday through Friday
Responsibilities:
Support and encourage individuals to increase their ability to be as independent as possible.
Meet with waiver participants on a regular basis- either in the individual's residence, the community, or an office setting.
Collaborate with the Case Manager to develop and facilitate programs which meet the social, emotional, physical and personal needs of the individual.
Provide ongoing support for integration efforts.
Document individual's progress toward acquiring community integration.
Assist in building a support network between the individual and the community.
Ensure medical consultation forms are completed, by the physician, for each medical appointment attended.
Administer medication(s) as needed and ensures required documentation is completed.
Coordinates all leisure and volunteer opportunities.
Minimum Requirements:
High School Diploma or GED Bachelor's Degree preferred Valid driver's license with a satisfactory driving record Minimum 2 years of relevant experience supporting individuals with intellectual and developmental disabilities Ability to establish a comfortable and supportive relationship with individuals receiving supports Ability to exercise good judgment and remain calm in crisis situations Excellent verbal and written communication skills Excellent time management skills Why Community Options?
